Abstract

The drilling machines (Hole, Bumpint, and Chanel) used in the Joint Plate production process at PT XYZ experienced recurring malfunctions from April through June 2025, with a total downtime of 6,820 minutes. This was caused by a maintenance system that only addressed issues after breakdowns occurred, without a well-planned preventive maintenance schedule. As a result, the production process faced disruptions, and the company faced the risk of financial losses. To address this issue, this study employed a quantitative approach combining Fishbone Diagram analysis (4M+1E) to identify the root causes of the failures, along with the Reliability-Centered Maintenance (RCM) method—which includes Failure Mode and Effect Analysis (FMEA)—to calculate the Risk Priority Number (RPN). In addition, Logic Tree Analysis (LTA) was used to classify the types of failures in each critical machine component. The results of the study show that the three main components that experience failures most frequently are the clutch, which failed 27 times; the punching system, which failed 21 times; and the hydraulic system, which failed 20 times. Based on the RPN value, the hydraulic system ranks highest in risk priority with a value of 441 and is categorized as a Category A safety issue. Therefore, it is recommended to implement both condition-based and time-based maintenance policies for these critical components in order to improve the overall reliability of the machinery.
