Occupational safety and health (OSH) is essential to prevent workplace accidents in industrial activities. CV. Dea Ayu Lestari, a solid waste collection and sales company, involves intensive manual material handling that poses significant accident risks. This study aims to identify potential hazards and analyze risk levels in waste processing activities using Job Safety Analysis (JSA) and Failure Mode and Effects Analysis (FMEA). JSA was applied to qualitatively break down work steps and identify hazards, while FMEA quantified the risks using Severity (S), Occurrence (O), and Detection (D) parameters to determine the Risk Priority Number (RPN). The results revealed that high-priority risks are concentrated in handling wooden and iron sheet pallets, with the highest risk occurring during the transportation of wooden pallets to collectors due to the hazard of being struck by collapsing materials, yielding a peak RPN of 126. Other critical risks include sharp metal cuts from rusted iron sheets (RPN 112). Recommended control measures include the operationalization of four newly designed Standard Operating Procedures (SOPs), the introduction of mechanical handling aids, and mandatory compliance with standardized personal protective equipment (PPE), specifically cut-resistant gloves and safety shoes.
